5/30/2023 EnchantaVaughn Approved 1.Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her overall disposition. QMHPA inquired about her morning. Kaila discussed with QMHPA that she was doing well on this morning. Kaila did not present any symptoms of S/I or H/I at the time of the session. 2.Behavior: Kaila at times was observed displaying hopeful behaviors evident by her stating to QMHPA on how she is glad to be able to have support. 3.Behavior: Kaila appeared to be a little depressed at times during this portion of the session as she reported to QMHPA on how she just needs to learn how to manage her depression daily. 1.Intervention: QMHPA began the session by inquiring about Kaila current thoughts, feelings and emotions. QMHPA challenged Kaila to identify her current level of anxiety. QMHPA engaged Kaila in a skill building activity. QMHPA reviewed the activity with Kaila. QMHPA challenged Kaila to identify ways she copes with her anxiety. QMHPA engaged Kaila in discussion surrounding identified ways she copes with anxiety. QMHPA engaged Kaila in an article entitled âUnderstanding Your Anxietyâ. QMHPA informed Kaila that the purpose of this session was for Kaila to gain insight on positive ways to cope with anxiety. 2.Intervention: QMHPA provided Kaila with another skills building activity. QMHPA informed Kaila that the purpose of this activity was for her to identify events that led up to her anxiety and depression. QMHPA engaged Kaila in discussion surrounding the importance of utilizing positive coping skills in an effort to decrease anxiety. QMHPA engaged Kaila in a discussion surrounding her medication management. QMHPA conducted a skill building activity with Kaila to assist her in developing an awareness of her depressive thoughts and actions as it relates to her diagnosis. 3.Intervention: QMHPA informed Kaila that the purpose of this activity was to assist her with reducing stressors that causes her to become depressed. QMHPA conducted another activity to assist Kaila with developing healthy coping skills that minimize her tendency to isolate. QMHPA informed Kaila that the purpose of this session was to encourage her to implement these into her daily living QMHPA taught Kaila 3-4 positive alternatives to practice to reduce depression and anxiety. QMHPA ended the session by reviewing the session and interventions discussed while also scheduling her next session three sessions for the week. 1.Response: Kaila appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Kaila communicated â1â as her current level of anxiety and stress level. Kaila expressed to QMHPA that it is low right now because her siblings has not gotten on her nerves as of yet. During the activity and the reading of the article, Kaila was able to identify ways she copes with anxiety experienced. Kaila also demonstrated the ability to communicate positive coping skills in an effort to decrease anxiety which was discussed during article. 2.Response: Kaila communicated the importance of utilizing positive coping skills in an effort to decrease anxiety as âto help control it as much as you canâ. Kaila maintained a positive attitude throughout todayâs session, displaying minor anxiety, distraught responses and apprehensive composure in regards to accomplishing goals in accordance to her treatment plan. During the skills building activity, Kaila was able to explain that coping thoughts are especially helpful when you first notice that youâre feeling agitated, nervous, angry, or upset. 3.Response: Kaila agreed with QMHPA and expressed that she did not use self-encouraging coping thoughts or self-monitoring to assist her with handling distressing situations. She reports at times just feeling so low because of the way the cancer has gotten her down; however, she is thankful to have someone to be able to motivate her to continue to be strong and not give up on a daily basis.
